7. Form 4: Pharmacy Cost Center

Form4 is completed in the same fashion. Look at Form4 in the Excel file. A few notes about entering information for the pharmacy cost center:

  • Relative values for pharmacy are based on the cost, or average cost, of the pharmacy supplies.
  • The “supplies” spreadsheet is used to develop the average cost.
  • If the program does not use one or more of the items listed, it is necessary to have a “zero” for the cost on column B. This will insure that Form 4 completes correctly.
  • The tab “Orals” provides a method for “weighting” pills that are used in greater quantity. This insures that lower priced pills do not reduce the average cost. It is also possible to differentiate between lower priced and higher priced pills.
  • The relative value for each of the supply codes is inserted automatically from the “Supplies” spreadsheet.
  • The next step is to insert utilization numbers for each of the supplies for the time frame under analysis.
