Form2 is completed in the same manner as Form1. Look at Form2 in the Excel file. A few notes about entering information for the counseling cost center:
- The activities associated with specialized counseling i.e. nutrition, smoking cessation, etc. are captured here, not routine counseling provided to all patients as part of a comprehensive visit. This is preventive counseling around a specific area designed to change behavior and/or reduce risk.
- The counseling must be documented in the chart and is recorded in fifteen-minute intervals.
- If programs do not track this counseling, it must be included in the medical cost center.
