Description
Audio conferences have been scheduled for November 20th and
December 16th, to introduce the new Division of Public
Health (DPH) STD Morbidity Report form and to address its use in
combination with the Centers for Disease Control and Prevention
(CDC) Field Record form.
Part I: November 20
The first audio conference
will focus on the revised STD Morbidity Report form (DPH 4243),
which is going into effect in November. Staff from the STD unit will
also touch on the companion CDC Field Record (CDC 2936). During
the broadcast participants will be briefed on the scope of reportable
STDs, the reporting process, and the steps DPH would like followed
for transitioning in the new form. Additionally, instructions for
completing and using the forms will be provided.
Part II: Dec. 16
By the time of the
second audio conference, use of the revised form will have begun.
This audio conference will provide an opportunity to follow-up,
recap, and receive feedback about the use of the form. CDC
Form 2936 will also be more fully discussed.
Target Audience
These two audio conferences are intended for all local public health
departments, Family Planning agencies, STD clinic personnel, and
other staff from related agencies who have any degree of
responsibility for reporting or follow-up on reportable sexually
transmitted diseases.
Featured Presenters
- Loriann S. Wunder, C.H.E.S.
- is a Public Health Educator for the
STD Control Section of the DPH. She has been working in
sexually transmitted disease intervention programs since 1990.
She is a Certified Health Education Specialist with a B.S. in
Community Health Education. She has trained various agencies
throughout the state in STD Control strategies.
- Alan Locke
- is a Public Health Associate for the CDC since 1988
and for the past five years has been assigned to work as the Data
Manager of the STD Control Section at DPH. Alan has been
instrumental in the plan to revise the STD Morbidity Report form.

Audio conferencing allow participants easy access to important topics without having to travel far or be out of the office for an extended period of time. And when you choose to listen in from a County Extension site, there is no charge!
Steps to participate:
- Register with HCET.
- Reserve a location that is quiet and convenient, such as a County Extension site or your own office conference room
(list of County Extension sites and phone in PDF
format).
- Dial in to the number provided and enter the conference code when prompted.
- Watch for the handouts and dial-in number to be sent to you 2-3 days prior to the broadcast.
