Accurate charting and documentation is a skill anyone can learn and practice. This skill is extremely important with all the new legal and reimbursement ramifications that can occur. This HCET Learning Link audio program will provide the listener with an increased awareness about both the medical and legal issues surrounding documentation of patient medical records. There will also be examples of SOAP Charting (Subjective, Objective, Assessment, and Plan) discussed, specific to your family planning visits.
Objectives
- Describe a client – centered discussion regarding Chlamydial infection, which includes answering client
concerns appropriately and correctly.
- To show an increase in awareness of the medical-legal environment
- To describe the importance of medical records as a legal document
- To list the principles of correct charting
- To discuss the essential elements of informed consent
- To show participants examples in charting and documentation
Presenter
- Barbara Jane (“B.J.”) Bacon
- Barbara has been a registered nurse for over twenty-five years. She spent 7 years in Clinical Management with Planned Parenthood of Wisconsin in the early 80's and 13 years with Planned Parenthood of North Texas. She worked as an Associate Project Coordinator for the Center for Health Training, through the Texas Department of Health, providing training to Title X family planning centers on a variety of subjects, including clinic management, contraceptive overviews, stress and time management, and creative problem solving. She has recently returned “home” to Wisconsin and Planned Parenthood of Wisconsin and serves as Vice President for Patient Services.
Participation

November 8, 2004. You MUST include an email address.
- Choose a location that is quiet and convenient for you to call in.
- Watch for the handouts & toll free dial-in number to be sent to you via email 2-3 days prior to the broadcast.
- Distribute and/or copy handouts to all
those who plan to listen.*
*We encourage the use of speaker phones so you can have multiple listeners.
The registration fee is only based upon number of telephone lines used by an individual or agency.
