This HCET Learning Link audio program will provide the listener with an increased awareness about both the medical and legal issues surrounding documentation of patient medical records. There will also be examples of SOAP Charting (Subjective, Objective, Assessment, and Plan) discussed, specific to your family planning visits.

Objectives

  • Describe a client – centered discussion regarding Chlamydial infection, which includes answering client concerns appropriately and correctly.
  • To show an increase in awareness of the medical-legal environment
  • To describe the importance of medical records as a legal document
  • To list the principles of correct charting
  • To discuss the essential elements of informed consent
  • To show participants examples in charting and documentation
